Required documentation for newly hired employees

At the time of hire, employers are legally required to have the newly hired employee complete two forms:

  1. federal Form W-4
  2. federal Form I-9.

The versions of both forms are current as of the date of publication and are included under the forms and policies tab at the top of this chapter. Effective October 21, 2019, U.S. Citizenship and Immigration Services (USCIS) issued a revised I-9 Verification Form that is valid until October 31, 2022. Employers may also visit the USCIS and Internal Revenue Service’s (IRS) websites to obtain copies of the current forms:

In addition, Oregon employers must submit an “Oregon New Hire Reporting Form” to the Department of Justice within 20 days of hire or rehire of an employee. The form may be found at:

Tax withholding

The Form W-4 is utilized by the employer for payroll purposes in calculating both federal and Oregon tax withholdings. The form is also available in Spanish at:
